Hypoglossal Canal Dural Arteriovenous Fistula Presenting as Ocular Symptoms

Abstract
Dural arteriovenous fistula (DAVF) within the hypoglossal canal is rare. Clinical symptoms of hypoglossal canal DAVF depend on the pattern of venous drainage, and pulsatile tinnitus is the common manifestation due to anterograde venous drainage into the jugular bulb. We report a patient with hypoglossal canal AVF presenting as chemosis and exophthalmos. A transfemoral cerebral angiography demonstrated a DAVF around the left hypoglossal canal, fed by the meningeal branches of left ascending pharyngeal and occipital artery, and retrograde drainage into the superior ophthalmic vein via cavernous sinus as well as anterograde drainage into the internal jugular vein. Our case showed that hypoglossal canal DAVF can cause ocular symptoms via retrograde venous drainage.
